Proof of Business Address

This document is used to verify that your business is in an 📍ELIGIBLE GEOGRAPHY.

• This Proof of Business Address document must: 

  • ◾◾ MATCH the Business Name and address listed on your application, Or the applicant’s name, if you are a Sole Proprietor

• Acceptable Proof of Business Address Documents include: 

  • Most recent tax return 

  • Business Utility bill within the last 90 days of the final application submission date (Acceptable examples: landline phone, electricity, water/trash, gas, TV, internet, or insurance) 

  • Current lease agreement (signed, dated) or current mortgage statement 

  • Business or professional licensure, not personal (needs an official “seal of approval” from the State of Minnesota) 

    • You may have a professional licensure if you are: a food vendor, bar owner, architect, chiropractor, surgeon, accountant, teacher, mechanic, barber or plumber, etc.

  • Secretary of State Certificate of Registration (screenshots of the Secretary of State registration page will not be accepted) 

  • If you are using a co-working membership as your business address, contact our team.

  • Bank statement
