Proof of Business Operating Address      

This document is used to verify that your business is in an 📍ELIGIBLE GEOGRAPHY.

  • This Proof of Operating Address document must: 

    • ◾◾ MATCH the Business Name and address listed on your application, Or the applicant’s name, if you are a Sole Proprietor

  • You must submit at least two documents to verify your eligibility. This includes one document that proves your business address and one document that proves your business operations.

  • Acceptable Proof of Business Address Documents include: 

    • Most recent tax return 

    • Business Utility bill within the last 90 days of the final application submission date (Acceptable examples: landline phone, electricity, water/trash, gas, TV, internet, or insurance) 

    • Current lease agreement (signed, dated) or current mortgage statement 

    • Business or professional licensure, not personal (needs an official “seal of approval” from the State of Minnesota) 

      • You may have a professional licensure if you are: a food vendor, bar owner, architect, chiropractor, surgeon, accountant, teacher, mechanic, barber or plumber, etc.

    • Secretary of State Certificate of Registration (screenshots of the Secretary of State registration page will not be accepted

    • If you are using a co-working membership as your business address, contact our team

  • 🚫 WE CANNOT ACCEPT

    • A driver's license

    • Driver's license 

    • Mobile phone utility bill 

    • Bank statement 

    • Screenshots of email confirmations

  • 💾 SAVE your changes on your application, finish ALL needed changes before you hit ✅ SUBMIT.

  • Acceptable Proof of Operations Documents include:

    • Business Utility bill within the last 90 days of the final application submission date (Acceptable examples: landline phone, electricity, water/trash, gas, TV, internet, or insurance)

    • Payroll records (within the last 90 days

    • A current lease agreement (signed and dated) or a current mortgage statement 

Business owners with a co-working membership that use the space’s address as their primary business location should contact our team for specific instructions and documentation requirements.
