Proof of Business Operating Address
This document is used to verify that your business is in an 📍ELIGIBLE GEOGRAPHY.
This Proof of Operating Address document must:
◾◾ MATCH the Business Name and address listed on your application, Or the applicant’s name, if you are a Sole Proprietor
You must submit at least two documents to verify your eligibility. This includes one document that proves your business address and one document that proves your business operations.
Acceptable Proof of Business Address Documents include:
Most recent tax return
Business Utility bill within the last 90 days of the final application submission date (Acceptable examples: landline phone, electricity, water/trash, gas, TV, internet, or insurance)
Current lease agreement (signed, dated) or current mortgage statement
Business or professional licensure, not personal (needs an official “seal of approval” from the State of Minnesota)
You may have a professional licensure if you are: a food vendor, bar owner, architect, chiropractor, surgeon, accountant, teacher, mechanic, barber or plumber, etc.
Secretary of State Certificate of Registration (screenshots of the Secretary of State registration page will not be accepted)
