Prepare for Round 2: Required Documents
Get a head start on your Round 2 application by gathering the documents listed below. These are documents we know will be required for Round 2. Preparing them now will help you complete your application more quickly once Round 2 officially opens. After the current legislative session ends on May 19th, additional documentation may be added to this list.
Documents to Start Preparing
Current (not expired) Photo ID
State or Federal-issued ID (e.g., driver’s license or passport)
Proof of Operating Address (One of the following documents - and no others - will be accepted):
Most recent business tax return
Utility bill (e.g., phone, electric, water)
Business lease
Business license
Complete and Filed Tax Return
Most recently completed tax year and 2021.Voided Check
For direct deposit verification. It must be a bank-provided paper check that includes the business name, account number, routing number, and bank institution. Applicants must upload images of both the front and back of the check.
Please note: We will not accept direct deposit forms.W-9 Form
Automated Clearing House (ACH) Document
Grant Agreement (will be provided to you)
Certificate of Grant Recipient (will be provided to you)
✅ If You Need to Show a Permanent, Fixed Business Location one of the following documents will be required:
A commercial lease agreement
Your most recent year of tax returns showing a commercial address
A signed and completed attestation form (will be provided to you)
